description: <div style='text-align:Left; font-size:12pt;'><p><span>UIC Facilities: Layer includes facility name and location, well classification(s), permit number, permit expiration date, ownership type and authorization status.</span></p><p><span>UIC wells have attributes for well site classification (Class V, IV or I) operational status, waste stream and type of well.  </span></p><p>UIC Well status: this table summaries the number of wells at the facility by well site (Class V,IV,I) and operational status. </p><p><span>The facility locations are based on a single well for that facility.  Facilities with no located wells are placed at  -83 longitude, 41.75 latitude.  Once a well is sited for a facility, the location will be updated to a well location.   The location for null points for UIC facilities is in Lake Erie, directly north of Camp Perry, Ottawa County.</span></p><p><span>Data Source:  UIC database, Division of Drinking and Groundwaters, Ohio EPA</span></p></div>
